What
does reduced-fee-for-service mean?
It is a plan in which dentists agree to accept lower fees for services
as payment in full.
What does PPO mean?
Preferred Provider Organization. FDH providers follow a contracted
fee schedule for the service they provide.
What does EPO mean?
Exclusive Provider Organization. The FDH EPO program, provides the
patient with a greater reduced fee for service, in this smaller,
exclusive network. The employer must have chosen the EPO program
and the provider must participate in the EPO network for patients
to receive benefits.
Who uses FDH?
FDH is a portable network aligning itself with several different
insurance companies, plan administrators and employer groups.

Can a patient see a specialist without first
seeing a general dentist?
Yes
Can a patient change dentists if they are unhappy
with their present one?
Yes, you can go to any dentist at any time as long as they are in
the network chosen by your employer (i.e. PPO or EPO).
Can a patient see a dentist if he/she is out
of network?
Depending on the plan design chosen by the employer, the patient
may or may NOT have benefits "out of network". The employee's Human
Resource Department will know this information.
How can a patient nominate their dentist?
By completing a nomination form online
or by calling FDH, at which time an FDH representative will contact
the dentist and send an application packet.
