What is FDH?
FDH is an independent, portable network. FDH aligns itself with over 40 insurance companies, plan administrators, and employer groups, providing a network of quality dentists as well as claims review. We are not an insurance company or an HMO.

How do we know which groups FDH is contracted with?
FDH distributes an Eligibility and Benefit Information Reference Guide to all of our providers, indicating which groups we are contracted with.

How do we find out information about benefits and eligibility?
The Eligibility and Benefit Information Reference Guide lists employer groups with a phone number to contact or refer to the back of patient's id card.

Do we need a special claim form?
FDH has its own claim form with all the necessary information. You can find this in the back of the policy and procedure manual. If you do not have an FDH claim form, any ADA form may be used.

Can we send claims electronically?
At this time, FDH is not equipped to receive claims electronically.

If you do not determine benefits, why do you need to see our claims?
Some employer groups have chosen utilization review as an option. In these case, the FDH Claims Analysts' review each claim for dental necessity. Then your claim is sent directly to the payor for benefit determination and payment.

How will patients know which dentists are FDH providers?
FDH produces a complete directory. FDH also provides a monthly provider update on disk to all contracted groups. You may also use our website - www.firstdentalhealth.com.

Do you accept all dentists?
FDH has credentialing standards in which all providers must reach. In addition, there must be geographic necessity as well as a need for the specific provider type in a certain area.

Are there any administrative fees associated with FDH?
No

What must a provider submit to join the FDH network?
A Completed provider application and agreement, as well as a copy of their wallet size license and malpractice insurance.

How long does it take to become a provider after submitting the requested information?
Providing all the information is complete, FDH accepts providers on a daily basis.
